Specimen Delivery, Requirements and Requisitions
All specimens must be accompanied by a requisition.
- From within Stanford Hospital and Clinics or Louise Packard Children's Hospital, contact your clinic nursing supervisor or call 723-6111 if you need a requisition
- From outside, please call 650-736-4207 for instructions for submission
